FOR PHYSICIANS

We are excited to welcome you to the Duke Health Integrated Practice (DHIP), the provider practice of Duke Health. When you join DHIP, you are joining an organization that drives clinical discovery and medical breakthroughs, teaches and trains the next generation of care teams, and delivers high quality, compassionate care.

What You Need to Do

Activate your Duke credentials

  • Be on the lookout for an email from help@duke.edu with the subject line: Duke University Guest Account with instructions for activating your Duke credentials and setting up Multi-Factor Authentication (MFA).
  • Make sure to activate your Duke credentials and upload your new badge photo. We are extending the completion date for this step to Friday, March 14.

Upload your badge photo

  • Visit the Duke Card website and log in with your Duke credentials from the NetID process (above).
  • Follow the instructions to upload a photo for your badge.
  • Make sure to complete this step by Tuesday, March 18, so your new badge is ready by April 1.
  • Duke Badges will be delivered to you on April 1st, but please keep your Lake Norman badge to maintain access.
  • All employee badges will display your legal first and last name.

Release of your employee health records

  • Your Lake Norman occupational health record will be transitioned to DUHS on April 1. No action is necessary unless you intend to opt out of the transition of your medical records.
  • If you choose to opt out of the automatic transition of your occupational health records, please contact lakenorman@duke.edu by Tuesday, March 18. When you opt out of this automated process, you will be required to supply additional information.
  • If you have questions about this transition, please contact Employee Health at EOHClinics@duke.edu.

What You Need to Know

On April 1, DHIP will assume your current CHS contract. During the months of March or April, we will schedule time with you for individual conversations on your DHIP contract terms. We will formally notify you of your new contract terms on May 1, beginning the 90-day notice period. Our intention is to come to terms by July 1. During these meetings, we will share more information on benefit plan details and how to make your selections, as well as next steps for other onboarding activities.
